Job Description

 

The Manager of the Care Management supervises and coordinates all activities related to the department which includes Disease Management and Ambulatory Care Managers. The Manager assists Nurse Manager(s)/Staffing Coordinator(s) in determining staffing requirements; designs, develops and implements plans for the advancement of the department and growth of the staff.  The Manager develops goals and objectives that enhance financial viability of the department.

 

Qualifications

 

Two years of college or an Associate’s Degree in Business, Healthcare, or other related field required. Bachelor’s degree in nursing or healthcare related field or current equivalent related work experience preferred. At least five years progressive experience in the health care or similar service industry with at least three years supervisory responsibilities and/or at least three years management experience at a middle management level. Current, valid California RN license; CPR certification preferred. Must be proficient in the use of Microsoft Word, Excel, Outlook and the Internet. Excellent written, verbal and interpersonal communication skills. Demonstrates knowledge of computer software programs utilized by the NAU and Disease Management Staff. Understands and is able to demonstrate NAU Triage activities performed by the staff (i.e., documentation, explanations, information gathering, teaching and disposition of information, etc.). Understands and is able to evaluate clinical assessment skills used in patient triage (acute conditions) and in telephonic evaluation and management of patients with chronic disease.
